If you require medical assistance please ring 111(please note this is a free call) or you can attend the walk-in clinic or the minor injury units, the details for which can be found in the above "opening times" tab.
Any local chemist can also provide advice and help as well.
Patients should only call 999 or attend A & E in a genuine medical emergency.
The staff at Shadwell Medical Centre in Leeds are proud to offer the highest standard of patient-centred healthcare. We run many clinics for the management of chronic diseases such as asthma and diabetes and offer a wide variety of other medical services including antenatal and postnatal care, childhood vaccinations and well-person check-ups.

Fax Machine: We no longer have a fax machine in the surgery . Patients can email us on email@example.com please note this email is constantly monitored.
eReception: is available 24 hours a day 7 days a week to enable patients to contact the practice. We would normally reply to you by text, e-mail, or telephone with a resolution to your enquiry. Online Access to your medical records changed from the 31st of March 2016. If you are registered for online services you can order repeat prescriptions, book GP appointments and view a summary of your medical records online. From 31st March 2016 you have beeen able to see more information including illnesses, immunisations and test results.
If you would like to register for online access please either click on the "online services" link on the right hand side of this homepage or pop into reception for a registration form. Please note that we will need to see photo identification (driving licence or passport) before we can complete registration. Any queries? Please see the below link or ask in reception!
