COVID -19 (CORONAVIRUS)
What to do if you have symptoms
Stay at home for 7 days and self isolate if you have either:
1. A high temperature
2. A new continuous cough
Do not come to the Surgery, Pharmacy or Hospital
You do not not need to contact NHS 111 to tell them your're staying at home
Use the NHS 111 online coronavirus service if:
1. You feel you cannot cope with your symptoms at home
2. Your condition gets worse
3. Your symptoms do not get better after 7 days

The staff at Shadwell Medical Centre in Leeds are proud to offer the highest standard of patient-centred healthcare. We run many clinics for the management of chronic diseases such as asthma and diabetes and offer a wide variety of other medical services including antenatal and postnatal care, childhood vaccinations and well-person check-ups.

Saturday and Sunday morning face to face appointments
There are now routine appointments available on Saturday and Sunday morning from 9am-12pm available through our extended access service at:
Rutland Lodge Medical Practice, Scott Hall Road, Leeds, LS7 3DR
This increased availability is an extension of the services offered by your practice and is provided by a range of local clinicians including GPs, nurses and health care assistants.
Please note, this is not a walk-in service – you still need to make an appointment. Appointments can be made either via your own GP practice or by calling the extended access team on 0113 221 3540 between 9am and 12pm on Saturday and Sunday.
Extended Access appointments are for routine general practice issues and not for urgent care. If your practice is closed and you are unwell please call NHS 111.
For NON URGENT enquiries & to save you time queuing when the telephone is busy
eReception: is available 24 hours a day 7 days a week to enable patients to contact the practice. We would normally reply to you by text, e-mail, or telephone with a resolution to your enquiry during practice hours. Weekend submissions will be dealt with on Monday morning. New Patient Registration
The practice is happy to register new patients living within our catchment area (see below). If you are already a patient and are moving locally please contact reception for advice. You may still be eligible to stay with the practice.
We will need to see you in person and you must present some form of identification to confirm your address, this is usually a utility bill or any other official document with your address on.

Registration usually takes up to 48 hours to be put on the system. After this you will be able to book appointments.
Each family member will need to register individually.

A virtual mental health support service (Healthy Minds)
This is a telephone appointment for patients aged 18 and over who would like help with emotional wellbeing. It will provide a 20 minute telephone consultation with Healthy Minds Practitioners who will be able to offer support for patients experiencing low mood, stress or anxiety. They will offer support, explore and develop coping strategies and offer signposting and recommend other support services. Please speak to the Reception team for further information on how to book an appointment.
CCTV is in operation at this surgery. The images are monitored for purpose of public safety, crime prevention, detection and prosecution of offenders. The scheme is controlled by Shadwell Medical Centre and for further information contact the Practice Manager.
Fax Machine: We no longer have a fax machine in the surgery . Patients can email us on email@example.com please note this email is constantly monitored.

The “Sharing Healthcare Records” leaflet was recently distributed to all practices across Leeds. Please take a look, the leaflet is aimed at patients to help you understand the different ways information is shared in Leeds. See: Sharing Health Records Leaflet
Online Access to your medical records changed from the 31st of March 2016. If you are registered for online services you can order repeat prescriptions, book GP appointments and view a summary of your medical records online. From 31st March 2016 you have beeen able to see more information including illnesses, immunisations and test results.
If you would like to register for online access please either click on the "online services" link on the right hand side of this homepage or pop into reception for a registration form. Please note that we will need to see photo identification (driving licence or passport) before we can complete registration. Any queries? Please see the below link or ask in reception!
