The doctors welcome new patients who live within our practice area. As it often takes some time for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the doctor. Please call in to the surgery to collect a registration form. We need one form per person wishing to register. This should be returned to the surgery prior to your New Patient Health Check.
When you register you will be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records
